From incident analysis to control system redesign improving control effectiveness in low voltage live work

Serious injury and fatality (SIF) prevention remains a priority across high-risk sectors such as mining. While most organisations maintain mature safety systems with documented controls, incidents persist – indicating that control existence does not guarantee control effectiveness. This case study examines Low Voltage (LV) live electrical work through the application of incident analysis and an evaluation of control effectiveness to better understand how controls perform under real operating conditions. Incident Analytics brought a structured and evidence-based approach combining detailed incident analysis with advanced semi-quantitative risk methodologies such as bow-tie modelling and control effectiveness assessments, consistent with critical control management guidance (ICMM, 2015a, 2015b, 2026). Historical incidents were analysed to identify where controls may erode or fail, enabling a deeper understanding of hazard pathways, erosion factors, and the real-world effectiveness of critical controls across the organisation. The project team comprised field operations, planners, technical training personnel and subject matter experts who translated incident analytics and assessment data into practical, actionable outcomes. The outputs included improved performance standards that identified where control health could be strengthened. Figure 1 presents the Control Effectiveness Assessment Framework used to evaluate the five factors that determine control effectiveness.
