The compliance paradox – why operational stability is the missing foundation for critical control management in mining
Starting 1 June 2026, all Queensland mines and quarries must include Critical Control Management in their safety and health systems. The intention is good, but the challenge is in how it will be put into practice. Most mines already try to run every part of their production chain at maximum efficiency. With the new verification rules, there will be more stoppages, more findings, and more pressure on supervisors who are already busy. Production targets will not change to accommodate this. This creates a compliance trap: a well-designed safety reform that, in an already stretched operation, can sometimes make the site less safe. This paper explains what happens, why it happens, and what can be done about it. The argument has three parts. First, the plateau in Australian mining fatalities, about seven deaths a year for the last decade, is not a behavioural issue, but a structural one. Second, the problem is how most mines are run: every resource is pushed to its limit, with no slack to absorb a stoppage. Third, the solution is also structural. We call it the Flow Layer: one constraint protected by buffers and spare capacity in every other resource, a small set of flow-based KPIs at the top of the daily report, and a single 30 min cross-functional meeting each day. In over 90 implementations across three continents, this approach has typically delivered 10–40 per cent more output from the same resources, along with the operational stability needed for the new CCM regime to succeed.
