Open pit mining operations have achieved exceptional capability in identifying, measuring, and controlling physical hazards during the past 30 years. Hazards such as Geotechnical stability, traffic interaction, mobile plant, and energy sources are managed through mature engineering processes, often supported by critical control frameworks and assurance protocols. Yet despite this technical sophistication, many serious incidents, near misses, and operational disruptions within open pit environments continue to originate not from failures of physical engineering, but from breakdowns within the human system that connects our physical controls. These remain poorly measured, weakly controlled, or normalised over time.
